Iridium Suite claim scrubber updated for the 2026 radiation oncology codes

News·

The Iridium Suite claim scrubber has been updated for the radiation oncology code changes that take effect on January 1, 2026. The update is in place for every practice, and charges with a 2026 date of service are checked against the new code set.

What billers will notice:

  • Deleted codes are stopped at entry. A charge for a code that no longer exists for a 2026 date of service, such as the old IMRT delivery codes, CT guidance or the freestanding G-codes, is stopped with a warning that names the codes that replaced it. The scrubber does not choose the new delivery level itself, because only the treatment plan shows whether a fraction is 77407 or 77412.
  • Image guidance is professional only. 77387 needs modifier 26 in every setting from 2026, including freestanding centers that bill everything else globally. The scrubber checks for it.
  • Weekly management follows the new codes. The 77427 fraction count now runs on 77402, 77407 and 77412.

Charges imported from the treatment system are checked the same way. If a charge crosswalk in Aria, Mosaiq or another system still sends a deleted code, the scrubber stops it at import, and we work through the table updates with each practice.
