77407 vs 77412 in Medical Billing: Choosing the Radiation Treatment Delivery Level in 2026

Medical billing terms· Reviewed October 2026

77407 vs 77412 is the choice between Level 2 and Level 3 radiation treatment delivery: 77407 covers single-isocenter photon treatment, 3D or IMRT, while 77412 requires multiple isocenters, active motion management, total skin electrons, or mixed electron and photon fields. Both include image guidance when performed.

The 2026 delivery codes are new for everyone. The level must be chosen for every fraction, so it is important to get the criteria exactly right.

Where you see it

Place What goes there
CMS-1500 box 24D and 24G 77402, 77407 or 77412 with no modifier, one unit per fraction; these are technical-only codes (PC/TC indicator 3) billed by the freestanding center
837P electronic claim Loop 2400, SV1 segment: the delivery code, unit count 1 per fraction
Hospital outpatient claim The same CPT code, paid under OPPS through APCs 5621, 5622 and 5623
Medicare Physician Fee Schedule (RVU26D) Status A, PC/TC indicator 3; 2.46, 9.28 and 13.28 total non-facility RVUs
Medicare MUE table (October 1, 2026) 2 units per date of service for each code
Treatment record Isocenter count, motion management technique and beam modality for each fraction; this is the documentation that supports the level

The three levels

The 2026 CPT descriptors, as CMS prints them in the CY 2027 proposed rule:

Code Level Descriptor
77402 Level 1 Single-electron field, multiple-electron fields, or 2D photons, including imaging guidance, when performed
77407 Level 2 Single-isocenter (for example, 3D or IMRT), photons, including imaging guidance, when performed
77412 Level 3 Multiple isocenters with photon therapy (2D, 3D or IMRT), or a single-isocenter photon therapy (3D or IMRT) with active motion management, or total skin electrons, or mixed-electron/photon field(s), including imaging guidance, when performed

Three things follow. Image guidance never changes the level, because every level includes it. The planning technique never changes the level, because Levels 2 and 3 both say “3D or IMRT”. And one Level 3 criterion is enough.

The single-isocenter rule

Before 2026 the IMRT delivery codes 77385 and 77386 (and G6015 for freestanding centers) paid more than 3D delivery, so “IMRT” on the plan meant the higher code. That is no longer true. The CY 2026 final rule calls the revised family “technique-agnostic”: a prostate VMAT plan, a head and neck IMRT plan and a free-breathing 3D lung plan are all 77407 if each uses one isocenter. Multiple targets covered from one isocenter are still 77407; the code reads “multiple isocenters”, not multiple targets.

Active motion management, total skin electrons and mixed fields

  • Active motion management is the Level 3 criterion practices meet most often: respiratory gating or breath hold for a left breast, lung or liver target. During rulemaking a commenter told CMS that if 77412 is “limited to cases involving breath-hold or gating technology” it would be about 15 percent of volume; that is the plain reading. Daily imaging, even with intrafraction tracking, is image guidance, not motion management.
  • Total skin electron therapy is Level 3 every fraction, even though it is electrons.
  • Mixed electron and photon fields means both modalities in the same session, for example photon tangents plus an electron boost field. Electrons alone are Level 1; the mix is Level 3.

Decision table

Scenario Code Why
Prostate IMRT or VMAT, one isocenter, daily cone beam CT 77407 Single isocenter, photons; imaging is included
Head and neck IMRT covering primary and nodes from one isocenter 77407 Multiple targets, one isocenter
Left breast tangents with deep inspiration breath hold 77412 Active motion management
Two bone metastases treated at separate isocenters in one session 77412 Multiple isocenters
Total skin electron therapy 77412 Named in the descriptor
Photon tangents plus en face electron boost, same day 77412 Mixed electron and photon fields
Single en face electron field to the skin 77402 Electron field(s)
Parallel opposed 2D photons to a painful spine 77402 2D photons

What the documentation must show

UnitedHealthcare’s commercial policy states the requirement in one sentence: when 77412 is reported, “documentation must clearly describe the circumstances that justify Level 3 rather than Level 2 treatment delivery” (Radiation Therapy: Fractionation, Image-Guidance, and Special Services, effective March 1, 2026). Medicare contractors have not published a 77412 checklist, so build your own from the descriptor. For every 77412 fraction the record should name the criterion: the isocenter count, the motion management technique actually used that day (not merely available on the machine), the total skin electron prescription, or the electron and photon fields delivered. Noridian’s radiation oncology page adds that voluntary breath holding is not considered appropriate for movement best managed with gating technology; if the plan says gating and the daily record says breath hold, expect the payer to question it.

Worked example: a 20-fraction prostate course and a 16-fraction breast course

Illustrative figures from the CMS RVU26D file at the 33.4009 conversion factor, national, before geographic adjustment and sequestration: 77407 is 9.28 RVUs, about 309.96; 77412 is 13.28 RVUs, about 443.56; 77387-26 is 1.09 RVUs, about 36.41.

Course A, prostate IMRT, single isocenter, daily cone beam CT, freestanding center. 20 fractions.

Line Units Allowed
77407 (technical, the center) 20 20 x 309.96 = 6,199.20
77387-26 (professional, the physician) 20 20 x 36.41 = 728.20

Had the center reported 77412 instead, the delivery lines would total 8,871.20, a difference of 2,672.00 with nothing in the record to support it. Because of that difference, payers audit the level.

Course B, left breast, deep inspiration breath hold, 16 fractions. Every fraction meets the active motion management criterion: 16 x 77412 = 7,096.96, plus 16 x 77387-26 = 582.56. If breath hold was abandoned at fraction 9, fractions 9 through 16 are 77407; the level follows what was delivered, fraction by fraction.

The CMS utilization assumption versus early 2026

The RUC crosswalk CMS proposed for 2026 assumed 45 percent of delivery would be 77412. Commenters said 15 percent. CMS finalized 55 percent to 77407 and 35 percent to 77412 (the remaining 10 percent falls to 77402) and priced the practice expense from OPPS APC weights on that mix. In the CY 2027 proposed rule CMS reported that in the first three months of 2026 77412 “comprised approximately 18 [percent] of the total volume of these services” and proposed to revalue the family on the observed distribution. In plain terms, the 2026 rates assumed more Level 3 than practices bill, and the 2027 rule would shift more of the payment to 77407. See the 2027 proposed rules for radiation oncology.

What to do

  • Set the level per fraction from the daily treatment record, not from the plan type in the charge master. The R&V system records the isocenter count and whether gating was used.
  • Keep a Level 3 justification on file before the first 77412 is sent. A one-line statement of the criterion in the physician’s setup note, repeated in the daily record, is the documentation an appeal needs.
  • Denials to recognize: CARC 151 with RARC N362 when more than 2 units post on one date; CARC 4 with N519 if anyone appends 26 or TC to a delivery code; CARC 16 with N56 or M51 when 77385, 77386, 77401 or a G-code is sent for a 2026 date of service (correct and resubmit); and a payer downgrade, which shows as 77412 paid at the 77407 amount, often with CARC 45 and no explanation. The downgrade is an appeal with the daily record attached; the others are corrections.
  • Iridium Suite’s claim scrubber warns when a deleted delivery, IMRT, guidance or port film code is billed for a 2026 date of service.

Frequently asked

What is the difference between 77407 and 77412?

Both are 2026 radiation treatment delivery codes that include image guidance. 77407 is Level 2: single-isocenter photon treatment, whether 3D conformal or IMRT. 77412 is Level 3, and it requires one of four things: multiple isocenters, a single isocenter with active motion management, total skin electron therapy, or mixed electron and photon fields on the same day.

Is IMRT billed as 77412?

Not by itself. Since January 1, 2026 the delivery level is set by what the delivery required, not by the planning technique. A single-isocenter IMRT or VMAT treatment with daily cone beam imaging and no motion management is 77407. It becomes 77412 only when a Level 3 criterion is met, such as breath hold or gating.

How many units of 77402, 77407 or 77412 can be billed per day?

One unit per fraction. The Medicare MUE for each of the three codes is 2 per date of service, which allows twice-daily treatment with a distinct break between sessions. The codes are technical-only (PC/TC indicator 3), so they never carry modifier 26 or TC.

What did CMS assume about how often 77412 would be billed?

In the CY 2026 final rule CMS priced the family assuming 55 percent of delivery would be 77407 and 35 percent 77412. In the CY 2027 proposed rule CMS reported that 77412 was about 18 percent of volume in the first three months of 2026 and proposed to revalue the codes on the observed mix.
