Hyperfractionation Billing: How to Bill Twice-Daily (BID) Radiation Fractions

Medical billing terms· Reviewed October 2026

Hyperfractionation billing is the reporting of a radiation course that delivers more than one fraction a day: each fraction is billed as its own delivery line, the second one on a date carrying modifier 76, and the management and physics units (77427, 77336) are counted by fractions delivered, never by days.

Twice-daily courses are uncommon, so a payer’s system or a new biller can mistake the claim for a duplicate. The rules are clear; apply them on each line.

Where you see it

Place What goes there
CMS-1500 box 24D, two lines with the same date in 24A Line 1: 77407, 1 unit; line 2: 77407 with modifier 76, 1 unit
837P Loop 2400, two SV1 segments with the same service date; the second with modifier 76 in the procedure code composite
CMS-1500 box 19 or 837P NTE segment The billing note on the week’s management line: “BID, AM/PM treatments” or “10 fractions”
77427 and 77336 lines Units from the fraction count: ten fractions in a five-day week is two units of each
MUE table (effective 10/01/2026) 77402, 77407, 77412: 2 per date of service, adjudication indicator 3 (clinical); 77770 to 77772 (HDR): 2
Record and verify system Two treatment records per day with times; this is the proof of a distinct break

What Medicare says

The Claims Processing Manual, Chapter 13, addresses both delivery and management:

  • Delivery (section 70.3): “Multiple treatment sessions on the same day are payable as long as there has been a distinct break in therapy services, and the individual sessions are of the character usually furnished on different days.”
  • Management (section 70.1): “Multiple fractions representing two or more treatment sessions furnished on the same day may be counted as long as there has been a distinct break in therapy sessions, and the fractions are of the character usually furnished on different days.”

“Of the character usually furnished on different days” is the test. A BID fraction is a full prescribed fraction, separated from the first by hours; the classic small cell lung schedule of 45 Gy in 30 fractions of 1.5 Gy twice daily is the standard example. A single fraction interrupted by a machine fault and finished an hour later is one fraction, not two.

The NCCI Policy Manual adds the brachytherapy case: partial breast HDR brachytherapy may be performed twice a day, and the HDR codes 77770 to 77772 carry a 2026 MUE of two per date of service. The external beam delivery codes 77402, 77407 and 77412 carry the same MUE of two, with a clinical adjudication indicator, so two lines on one date pass the edit and a third does not.

Modifier 76 and the second line

Modifier 76 is “repeat procedure or service by the same physician or other qualified health care professional.” Novitas’s modifier page says the circumstance “may be reported by adding the modifier 76 to the repeated procedure or service,” and that the repeat must be documented as a distinct service or it will be denied. For a BID fraction that means:

  • The first fraction on its own line with no modifier.
  • The second fraction on a second line, same date, same code, modifier 76, one unit.
  • No 26 or TC on either line. The level delivery codes are technical-only (PC/TC indicator 3), so 76 is the only modifier they carry.
  • Where the physician’s image review is reported with 77387-26 for each fraction, the second review follows the same two-line pattern; the 2026 MUE for 77387 is two per date.

Some payers accept a single line with two units and the modifier instead of two lines. Separate lines is the safer default, because the units field on a repeated service is what duplicate logic examines; a payer that wants one line will say so in its modifier fact sheet.

The counts: fractions, not days

Take 45 Gy in 30 fractions of 1.5 Gy, twice daily, over three weeks: fifteen treatment days, thirty fractions.

Line Units How
77407 30 lines fifteen plain, fifteen with modifier 76
77427 6 30 fractions divided by five; two units fall in each calendar week
77336 6 one per five fractions, same cadence

Noridian’s billing tip for the management code is explicit: “More than one set of treatments per day? NOS = 2 and narrative reflects one of the following: AM/PM treatments; Eight (8), nine (9), or 10 fractions; BID or twice a day.” Without the narrative, the second unit denies as a duplicate. At the end of the course the usual rule applies by fraction count: three or four leftover fractions earn a final unit, one or two do not, and Noridian asks that the narrative “list final three (3) or final four (4) fractions.”

Illustrative figures, 2026 national amounts before geographic adjustment (RVUs from the CMS October 2026 RVU file times the 33.4009 conversion factor): 77407 at 309.96 is 9,298.80 for thirty fractions, 77427 at 195.40 is 1,172.40 for six units, and 77336 at 92.19 is 553.14 for six units. The same thirty fractions given once daily over six weeks come to exactly the same amounts. What changes is the dates on the lines and the note that explains them.

Documentation

  • The prescription stating fractions per day, dose per fraction and the minimum interval between sessions.
  • Two treatment records per date with times, from the record and verify system, showing the break.
  • The physician’s note on why the schedule was chosen.
  • A management note for every billed unit of 77427 (two in a BID week) and the physicist’s signed check for every unit of 77336.

What to do

  • Post every fraction as its own line from the treatment record, and add modifier 76 to the second fraction on each date before the claim is sent.
  • Count 77427 and 77336 by fractions. Post the second weekly unit with the BID narrative in box 19 or the NTE segment.
  • Keep the modifier off the unit codes. 77427 and 77336 take units, not modifier 76.
  • Denials to recognize: CARC 18 (exact duplicate) on a second delivery line that went out without modifier 76, a correction and resubmission, not an appeal; CARC 151 with RARC N362 on a third delivery line on one date, or on 77427 units that exceed fractions divided by five, a correction unless the record shows three distinct fractions; CARC 18 or CARC 151 on the second weekly 77427 unit when the narrative was missing, a resubmission with the note or an appeal with the fraction log; CARC 4 when modifier 76 is added to 77427 or 77336, a correction; CARC 16 with RARC M53 when units and lines disagree.
  • In Iridium Suite, 77427 and 77336 can be created automatically from the fraction count imported through the Record & Verify integration, with a billing note.

Frequently asked

How do you bill two radiation fractions on the same day?

Each fraction is its own delivery line with the same date of service. The first carries the delivery code alone (for example 77407) and the second carries the same code with modifier 76, repeat procedure by the same physician. Medicare pays both when there was a distinct break between sessions and each fraction is of the kind usually given on separate days. The 2026 MUE for 77402, 77407 and 77412 is two per date.

How is 77427 counted for a BID course?

By fractions, not days. Ten fractions in a five-day week is two units of 77427 and two units of 77336. Medicare's manual says same-day fractions count as long as there was a distinct break and the fractions are of the character usually furnished on different days. Noridian asks for two units with a narrative such as BID or AM/PM treatments.

What is the billing note for BID treatment?

A short narrative on the claim (CMS-1500 box 19 or the 837 NTE segment) that explains the second unit: Noridian's examples are AM/PM treatments, BID or twice a day, or eight, nine or ten fractions. Without it, the second weekly unit looks like a duplicate.

Does modifier 76 go on 77427?

No. 77427 is a unit count, not a repeated procedure, so a second unit in the same week is simply two units with the narrative. Modifier 76 belongs on the repeated delivery line (and, where reported, on a repeated 77387-26 image review), never on the management or physics codes.
